School Nomination Request Form:

To be considered for the National Scholarship program a school nomination is required.

In this section, you are required to enter the email address of your school administrator (i.e.Principal, head of school or designate such as a Guidance counselor/officer).
This is your request for a nomination for the Western National Scholarship Program and it will be sent to the school administrator you provide. Your school administrator will be sent an email (once you click on the "send email" button) with your request for a nomination and request for transcript. Instructions to your school administrator on how to provide the nomination and transcript will be included in the email.

Students should complete the section "Nomination Request Form" in advance in order to give the school time to consider and complete their request for a nomination and provide a transcript. The deadline to submit the application (including nomination and all supporting documentation) is February 14, 2025.

Some schools have an internal process that students have to follow in order to be considered for a nomination. Please check with your school (e.g., Guidance/Student Services Office) to obtain information for any internal process you may need to follow.

If necessary, you will be able to resend the school nomination request 2 additional times. Once the request has been sent a total of 3 times you will no longer be able to resend the request. Once you send your request, you will be able to check the status by going to section 'Status' of your application.


Once the email is sent out, the student must follow up with the school administrator to let him/her know that the email has been sent. It is the student's responsibility to follow up and make sure their application is complete and submitted by the deadline.
